The Institute of Medicine Report To Err Is Human: Building a Safer Health System (National Academy Press, 2000) outlines the significant costs of medical errors that can be preventable. The patient and his/her caregivers have an important responsibility to actively participate in the health care process. In this section, the University of Arizona CERT provides tools to assist patients with safe medication use. We also provide links to other useful sites that give additional information on therapeutics. The goal of these efforts is to provide access to information that will help improve patient safety.
